Appointments may be scheduled by calling our centralized scheduling center at (818) 933-2580.

You will be directed to the appropriate office for your examination. For your convenience, we provide bilingual staff at all of our locations.

You may download a patient information form to fill out in advance of your appointment and either e-mail the completed form to appointments@svrimaging.com or fax to (818) 933-2581 which will expedite your sign in process the day of your examination.

If you prefer, you may hand carry the completed form with you and give it to the receptionist on the day of your examination.
